Senior Living Community
Heritage Heights (HH) is a licensed assisted living facility located in Chelan County in Northcentral Washington. The non-profit 501(c)3 operates a resident-centered facility that has been serving seniors since 1996.
Heritage Heights has a proven 28-year track record of providing 24/7 high-quality assisted living services to seniors and medically fragile in the region. We also provide comfortable community amenities such as a fireside living and game area, TV and movie room, and space for exercise classes and workout equipment.
Our staff have special certifications for providing comprehensive assisted living and respite care services, including personal care options such as medication management, bathing, housekeeping, and nutritional services.

Our roots and history are deeply embedded in our local community. Unlike for-profit facilities, we are primarily accountable to our community and its residents. We manage our pricing and expenses carefully so we can continue to accept Medicaid reimbursement for those who do not have other resources.
Meet our Staff
Maria Jay, RN
Executive Director – Heritage Heights Assisted Living
Maria Jay, RN, is the Executive Director of Heritage Heights Assisted Living and has proudly served the Heritage Heights community for the past three years. She began her journey at Heritage Heights as Acting Director of Wellness and now leads the community as Executive Director, combining strong clinical experience with compassionate, relationship-driven leadership.
Maria and her family moved to Chelan in the summer of 2020 after many years of visiting the area and falling in love with the community and its people. She has two young adult children, and her youngest is a graduate of Chelan High School. Outside of work, Maria enjoys golfing, pickleball, swimming in Lake Chelan, traveling, and spending time with family and friends. Raised in a military family, Maria had the unique experience of living in many places throughout the United States and abroad. She was born in Hawaii, graduated from high school in Germany, and ultimately chose Washington State to pursue her nursing education. After earning her nursing degree, Maria began her career in hospital-based care, working extensively in medical, neurological, and cardiac intensive care units. She has also served patients in outpatient cardiology, home health, and hospice settings.
Maria is deeply passionate about serving the Chelan community through Heritage Heights. She values the strong sense of connection between residents, families, and staff and is honored to lead a community that feels like home to so many. Supporting residents with dignity, compassion, and respect is at the heart of her leadership.
Autumn, CNA
Director of Wellness
Autumn Dial, CNA, brings more than 30 years of experience in caregiving and healthcare to her role as Director of Wellness at Heritage Heights. Having entered the field in high school, she has devoted her career to serving seniors with compassion, professionalism, and integrity.
Autumn has been a valued member of the Heritage Heights team for over 10 years and currently oversees care for residents in Assisted Living, Memory Care, and hospice services. She is deeply committed to supporting residents throughout every stage of aging—from independence through end-of-life—while ensuring dignity, comfort, and quality of life.
Her passion for long-term care is rooted in her belief that every resident deserves personalized, respectful, and consistent support. She is especially driven by the growing need for a full continuum of care within the local community and takes pride in helping families navigate each stage with confidence and peace of mind.
Born and raised in the Chelan area, Autumn is a graduate of Chelan High School, as are her children. Her lifelong connection to the community strengthens her relationships with residents and families alike.
Outside of work, Autumn enjoys spending time with her family, camping, hiking, and traveling—even for short getaways. She is proud to serve the community she calls home and considers it an honor to care for its seniors.
Laurie Holmes
Director of Life Enrichment
Laurie Holmes brings a deep passion for working with people and enriching the lives of seniors at Heritage Heights. She began her career in healthcare in a medical office and hospital admitting, where she discovered her love for helping individuals from all walks of life and ensuring they felt supported and welcomed.
Inspired to work more directly with people, Laurie attended Western Washington University at the Everett Community College campus, earning a Bachelor’s degree in Human Services. She went on to gain valuable experience providing direct care and supervisory support for adults with developmental disabilities and older adults.
Laurie has served in several roles supporting seniors, including Activity Assistant in a nursing home in Monroe and Activity Director at an Alzheimer’s Special Care facility in Kennewick. She also worked as a Care Coordinator with the Washington State Health Home Program, helping individuals navigate healthcare services, schedule appointments, and connect with vital community resources.
Laurie discovered Heritage Heights while working with clients in Chelan and later joined the team as the Director of Life Enrichment, serving both Assisted Living and Memory Care residents.

Our History
In 1995, the need for senior housing for those that could no longer remain independent in their own homes was apparent and immediate to a group of caring citizens. The community went to work raising funds to build the facility, our non-profit 501(c)3 was created and a community board was established. The facility was completed in early 1996 and the facility was full within 30 days.

The senior population continues to grow in our region and as a result families are having to send their loved ones at least 40 miles away to receive the care they need, taking them from the security of their doctors and the community they know and family they have come to rely on. This puts additional burden on our neighboring communities who are already struggling to meet the needs of their own fragile seniors and who may not accept Medicaid payment.
